How to enroll
- Choose your coverDecide which plan suits you and who in your family will be covered.
- Complete the enrollment formProvide proof of identity, proof of relationship and proof of address for each insured person.
- Submit with your first premiumBring the form, documents and your first month’s premium to the credit union.
